The First
Caseload

Hello!

My name is Tyler Keith, and I am a Licensed Clinical Social Worker, Certified Clinical Supervisor, and Clinical Training and Development Consultant based in North Carolina.

I created The First Caseload to bridge the gap between graduate training and actual practice. It helps graduate counseling, clinical social work interns, and early career clinicians design a Caseload Operating System that fits who they are, how they work, and the life they want to live.

2026 Membership Consultation Dates & Topics

  • In this consultation, we’re going to walk through how to create a space where clients feel safe, clear, and supported right from the start. You’ll learn how to guide the first four therapy appointments in a way that gives clients room to be themselves while still helping you stay on track and get what you need. We’ll also look at how your style, your setting, and the person in front of you all shape that first meeting. The goal is simple: help clients leave feeling understood and confident about what comes next, without the first four sessions feeling rigid or all over the place.

  • Not every intake goes smoothly and that’s okay. In this consultation, we’ll break down the common things that can throw you off early on, like clients who don’t want to be there, feel overwhelmed, struggle to stay focused, or come in with big safety concerns. You’ll learn how to stay steady, take the lead when needed, and still show up with care and respect. The goal is to help you handle these moments without freezing, overthinking, or losing direction so your sessions can still move forward in a clear and helpful way.

  • This consultation will focus on what to do in the last 10 minutes of an intake once you’ve heard parts of the client’s story and gathered the key details. You’ll learn how to quickly make sense of what matters most, name a starting focus, and map out a simple plan before closing the session so you and your client aren’t left guessing what session two will bring. We’ll use the First Four Arc tool to help you sketch out a clear direction for the next few sessions so you know exactly where you’re headed. This isn’t about getting it perfect. It’s about building momentum and giving your work a strong, steady start.t goes here

  • In this consultation we’ll talk about what it’s like to be a real person while also learning how to be a clinician. Early in your career, a lot is changing at once new jobs, big life transitions, relationships, and pressure to “get it right.” All of that comes with you into the room whether you notice it or not. You’ll learn how to stay aware of your own thoughts, stress, and reactions so they don’t take over the session. The goal isn’t to shut yourself off. It’s to understand yourself well enough that you can stay present, steady, and focused on your client without getting pulled off track by your own overwhelm, doubts, or urgency to help.

  • This consultation will shed light on how to stay grounded in growth instead of getting stuck in self doubt. Early on, it’s easy to feel behind or unsure of yourself, but it’s hard to stay stuck there when you have a clear plan for what you’re learning and why. You’ll learn how to use the Learning and Development Plan from the Caseload Operating System to track your strengths, choose what to work on next, and stay focused on building real skills over time. The goal is simple: shift from worrying about whether you’re doing it right to staying curious and committed to getting better.

  • In this consultation, we’ll focus on how to set clear expectations with clients in a way that feels respectful, honest, and easy to understand. You’ll learn how to introduce expectations early, come back to them when needed, and handle moments when clients feel unsure, push back, or get frustrated. We’ll also talk about how to ask clients what they expect from you so the relationship feels more balanced from the start. The goal is to help you be clear and steady without feeling rigid so both you and your client know what to expect moving forward.

  • This consultation will go DEEP as we’ll talk about how to guide clients toward consistent care without feeling pushy or uncomfortable. You’ll learn how to have real conversations about frequency, follow through, and why showing up matters for progress. We’ll also take an honest look at the ways we sometimes back off too quickly, not because it’s best for the client, but because it feels easier in the moment. The goal is to help you speak clearly, stay grounded, and address attendance as part of the work so you can support real change instead of working around it.

  • In this consultation, we’ll focus on how to take care of yourself in a way that actually lasts. You’ll learn how simple habits like reflection, gratitude, and connection with others can protect you from burnout and help you stay energized in your work. We’ll also introduce the Professional Quality of Life Scale (ProQOL) as a tool to help you better understand how your work is impacting you. The goal is to help you build routines that support your life outside of work so your career adds to your life instead of taking it over.
